The Handling of Personal Information

The Division of Disaster & Radiation Medical Sciences joint major, offered by the joint graduate schools of Nagasaki University and Fukushima Medical University Graduate School of Medicine (hereinafter “Joint Graduate Schools”), is in compliance with personal information protection laws and regulations and shall make every effort to protect personal information based on the policy below, together with the Nagasaki University Personal Information Protection Rules (hereinafter “Rules”) and the Nagasaki University Personal Information Management Guidelines.

Purpose for the Collection of Personal Information

The purpose for and use of personal information collected by the Joint Graduate School is in conformance with Paragraphs 1 and 2 of Article 1 (“Purpose”) of the Nagasaki University Personal Information Protection Rules.

Collecting and Retaining Personal Information

>The Joint Graduate School shall use lawful and fair means for the collection of personal information.
>The Joint Graduate School shall not retain personal information beyond the extent necessary for the performance of school business for purposes that shall be specified insofar as possible.
>The purpose for the collection of the information, and the entity responsible for managing the information, shall be indicated when personal information is collected.

The Use of Personal Information

>The Joint Graduate School shall not use personal information for any purpose other than that indicated at the time of collection, nor shall it provide it to any third party unless there is legal basis for such use.
>The Joint Graduate School shall not provide personal information to any third party without prior consent, except when required by law.
>The Joint Graduate School, when providing personal information to a third party or entrusting procedures involving the handling of personal information to a third party, shall conduct a thorough investigation of the third party’s security measures and its capacity for managing the personal information. The Joint Graduate School shall properly supervise the third party in its handling of the personal information.

The Management of Personal Information

The Joint Graduate School shall take appropriate information security measures to protect against unauthorized access, computer viruses, and so on, to preserve the integrity of the personal information in its possession. It shall also act to prevent its loss, destruction, modification, or disclosure.

The Disclosure, Revision, and Suspension of the Use of Personal Information

The Joint Graduate School shall confirm that any user of its website, in regard to his or her own personal information, has the right to request the disclosure, revision, or suspension of use of said personal information, and shall comply with such requests as quickly as possible and as set forth in the Rules.

Compliance with the Law and the Revision of Procedures

The Joint Graduate School, while complying with laws and other regulations concerning the protection of personal information, shall continually revisit its measures and procedures for handling personal information and make efforts to improve them.
